SportSlip is a telehealth platform that provides physician-verified sports and camp physical clearance letters. Complete a health screening questionnaire, record a short video of your child, upload your camp or school's form, and we'll fill it out and sign it — all from your phone.
Yes. Our physician conducts a comprehensive telehealth pre-participation evaluation that includes a validated health screening questionnaire (based on AHA and PAR-Q+ protocols), a video-based physical observation assessment, and immunization record review. Research shows this approach detects cardiac risk more effectively than a stethoscope exam alone.
SportSlip fills out your camp or school's actual form — the same form they gave you. Our physician signs it with their medical license number and NPI. You also receive a comprehensive SportSlip clearance letter. Most camps and schools accept telehealth-completed physicals.
The screening questionnaire takes 3-5 minutes. After payment, you'll upload a 30-second video of your child and a photo of their immunization card. Most clearances are issued within hours.

Pricing

QuickClear (Tier 1) is $49.99. If our screening identifies items needing review, Tier 2 is $69.99 (includes ECG cardiac screening) and Tier 3 is $89.99. Additional children in the same household are $29.99 each.
Your first child is full price. Every additional child in the same household is just $29.99. For example, 3 kids = $49.99 + $29.99 + $29.99 = $109.97 (vs. $228 at urgent care).
ECG cardiac rhythm screening is included in Tier 2. Tier 1 parents can add it for $19.99 after their clearance is issued. It uses your Apple Watch or Samsung Galaxy Watch — no extra devices needed.

Safety

If our screening identifies a critical concern, we will not issue a clearance. Instead, we provide a free referral letter recommending your child be evaluated by a specialist (e.g., cardiologist) before sports participation. Your child's safety always comes first.
Standard urgent care sports physicals rely on a brief stethoscope exam that has only 9% sensitivity for detecting conditions causing sudden cardiac death. Our validated questionnaire has 20%+ sensitivity, and our optional ECG screening (used in European programs) has 87.5% sensitivity. We screen more thoroughly, not less.
All clearances are reviewed by Dr. Adam Z. Kawalek, MD — a board-certified internist licensed in 30+ states. Dr. Kawalek personally reviews all flagged cases and signs every clearance letter.

Technical

Any smartphone with a camera. You'll record a 30-second guided video of your child standing, walking, bending, and turning. No special equipment needed.
You can enter immunization dates manually, or check with your pediatrician or state immunization registry. Some states offer online portals where you can download your child's immunization record.
